Neck Pain

Neck pain ranges from simple mechanical strain to cervical facet joint degeneration to nerve root compression (radiculopathy) — three genuinely different problems that can feel similar but call for different treatment.

Understanding Neck Pain

The cervical spine's seven vertebrae, discs, and facet joints support the head through an enormous range of motion, which makes it more vulnerable to both mechanical strain and degenerative wear. Postural strain — often from computer or phone use — is the most common cause and typically the most benign. Cervical facet joints can degenerate the same way lumbar facets do, producing a localized ache worse with extension or rotation. And a herniated disc or narrowed nerve foramen can compress a cervical nerve root, sending pain, numbness, or weakness down the arm.

Some patients also experience cervicogenic headache — head pain referred from upper cervical structures — which follows the same diagnostic logic as the rest of the neck.

How treatment typically progresses

Start with Conservative Care

Medications

NSAIDs and, for acute muscular strain, short-course muscle relaxants are typically first. Neuropathic agents (gabapentinoids) are added when there's a radicular, arm-radiating component.

Physical Therapy & Exercise

Postural correction and deep neck flexor strengthening address mechanical neck pain directly. The same directional-exercise principles that apply in the lumbar spine carry over here: extension often aggravates facet-mediated neck pain, while directional exercise can help radiculopathy, consistent with guideline-recommended conservative care.

Activity & Behavior Modifications

Ergonomic adjustments — monitor height, phone posture, supportive pillows — reduce the repetitive strain behind much mechanical neck pain, alongside general activity pacing while symptoms settle.

If Conservative Care Isn't Enough

Cervical Epidural Steroid Injections

For cervical radiculopathy, a cervical epidural steroid injection reduces inflammation around the compressed nerve root. This carries a more serious risk profile than the lumbar version of the same procedure, which is why technique and training matter especially here.

Cervical Medial Branch Blocks (MBB) & Radiofrequency Ablation (RFA)

For confirmed cervical facet-mediated pain, medial branch blocks followed by radiofrequency ablation follow the same diagnostic-then-ablative logic established in the lumbar spine, applied at the cervical level.

This is a logical progression based on your response to each step — not a jump straight to the most invasive option, and not a rigid ladder you have to climb regardless of your diagnosis.

What's the diagnosis?

Distinguishing mechanical or postural neck pain from facet-mediated pain (worse with extension and rotation, confirmed by a cervical medial branch block) from radiculopathy (arm-radiating pain in a specific pattern, a positive Spurling's test, confirmed on MRI).

What's the mechanism?

Three distinct mechanisms can produce neck pain: muscular or postural strain, degenerative cervical facet joint disease (the same arthritic process seen in the lumbar spine, at cervical levels), or compression and irritation of a cervical nerve root from a disc herniation or foraminal narrowing.

What are the options?

Conservative care for most acute and mechanical neck pain; cervical epidural steroid injections for radiculopathy; cervical medial branch blocks and radiofrequency ablation for confirmed facet-mediated pain.

Why this approach?

Because the right treatment differs substantially by mechanism, getting that distinction right up front matters — treating chronic facet-mediated pain indefinitely like a muscle strain, or vice versa, wastes time and doesn't address the actual pain generator.

What are the tradeoffs?

Cervical procedures demand more precision than their lumbar counterparts, given the proximity of the vertebral arteries and spinal cord — which is why they're performed with cervical-specific technique by fellowship-trained interventional physicians. Conservative care remains the right first step for most patients regardless of which mechanism eventually turns out to be driving the pain.

Prognosis

Depends heavily on which of the three it is

Most acute, mechanical neck pain — the kind brought on by posture, strain, or a stiff night's sleep — resolves within days to a few weeks with conservative care. Cervical facet joint pain behaves more like its lumbar counterpart: a chronic, degenerative condition that's very manageable with the right diagnostic-and-treatment sequence, but not one that disappears on its own. Cervical radiculopathy from a disc herniation generally follows a favorable course similar to lumbar sciatica, though the tighter space in the cervical canal means some cases warrant closer monitoring.
