Sacroplasty
A cement augmentation procedure using the same principle as kyphoplasty, applied specifically to stabilize sacral insufficiency fractures and relieve the pain of a fractured sacrum.

What it involves
Sacral insufficiency fractures — most often from osteoporosis — are a distinct fracture from a vertebral compression fracture, occurring in the sacrum at the base of the spine rather than in a vertebral body. Under fluoroscopic or CT guidance, a needle is directed into the fractured portion of the sacrum, and bone cement is injected to stabilize it.
The sacrum's anatomy — irregular, curved, and close to nerve roots exiting toward the legs — makes precise, careful needle placement especially important here, distinct from the more straightforward vertebral body access used in kyphoplasty.
How it works
Stabilizing the fracture with cement eliminates the abnormal micromotion at the fracture site that drives much of the pain — the same underlying principle as kyphoplasty, applied to the sacrum's distinct anatomy rather than a vertebral body. Unlike kyphoplasty, sacroplasty doesn't involve restoring height with a balloon or implant first, since sacral insufficiency fractures don't collapse the same way a vertebral body does.
A growing base of supportive evidence documents meaningful pain relief for appropriately selected patients with sacral insufficiency fractures, applying the same cement-augmentation principle established for vertebral fractures to this anatomically distinct area.

What's the diagnosis?
Correlating the onset and location of pain — typically low back or buttock pain, often in an older patient with osteoporosis — with imaging (MRI or CT) confirming a sacral insufficiency fracture, distinguishing it from a vertebral compression fracture or SI joint problem.
What's the mechanism?
Bone cement injected into the fractured portion of the sacrum eliminates the abnormal micromotion at the fracture site that's driving pain, stabilizing the bone directly rather than simply managing the pain around it.
What are the options?
Bracing, pain management, and osteoporosis treatment for fractures expected to heal on their own; sacroplasty for fractures causing significant, persistent pain that hasn't improved with conservative care.

What to Expect
An outpatient procedure with careful, image-guided technique
The procedure typically takes about 30 to 60 minutes, done under sedation or light anesthesia on an outpatient or short-stay basis. Given the sacrum's proximity to nearby nerve roots, Dr. Foster uses meticulous image guidance throughout to confirm safe, accurate cement placement.
Many patients notice meaningful pain relief within a day or two as the fracture stabilizes. Walking is generally encouraged soon after, with a gradual return to normal activity over the following days to weeks.
